After that, in the middle east swing, he fell in the second round to Dominic Thiem at the Qatar Open. And Karatsev eventually claimed his maiden ATP tour-level singles title at the Dubai Duty-Free Tennis Championships as a wild card entrant.

 At the Miami Open 2021, Russian lost his second-round match to host Sebastian Korda in straight sets. In Monte Carlo, he advanced into the round of 32 after defeating Italian NextGen Lorenzo Musetti without dropping a set.
